People on several or high-risk medicines, insulin, or new prescriptions.

Please have ready

Current medication list and any signed medication authority from your GP.

How care is delivered

A Registered Nurse visits you at home or in supported accommodation. Care is provided with your consent and within the nurse's scope of practice. If anything needs a doctor's review, we contact your GP with your permission.
