A full nursing assessment at home covering health history, medicines, skin, mobility and home safety, with a written care plan.

- Health history and current conditions
- Medication review against your pharmacy list or Webster pack
- Skin, wound and pressure-area check
- Mobility, falls risk and home safety check
- Cognition and mood screening
- Recommendations for equipment, products and allied health referrals where needed
- Written care plan for you and, with your consent, your GP
Who it's for
New clients, people recently home from hospital, or anyone whose needs have changed.
Please have ready
Medication list or Webster pack, recent discharge papers or GP letters, Medicare or NDIS details.
How care is delivered
A Registered Nurse visits you at home or in supported accommodation. Care is provided with your consent and within the nurse's scope of practice. If anything needs a doctor's review, we contact your GP with your permission.
